If you decide to reactivate your membership in the future (and we hope you do), membership rates in effect at the time of reactivation will be applicable as membership rates are subject to change. If you would like to put your membership on hold instead of cancelling, you can submit a Membership Hold Request for a 30, 60 or 90 day period.

1. If you’re sure you’d like to cancel, just complete and submit the form below at least 3 days prior to your next scheduled payment in order for the Exclusive Coaching Contract to be terminated effective as of the last day of the calendar month. All payments are non-refundable.

2. If the client terminates the contract during the time frame under Contract, the client will be required to pay Five Hundred Dollar ($500.00) cancellation fee for canceling during their contract.

3. Membership agreements are auto-renewed as month-to-month unless the MEMBERSHIP CANCELLATION FORM is submitted to us at least 30 days prior to your scheduled auto-renewal.
